"Additional Family Information" Form

On this form, you're asked to write the name in English and in your native language. Although I know Urdu, I have always indicated English as my native language on all previous forms and in this application as well. Do I only sign in English????

I don't think you understood my question. I'm asking if I must write in Urdu. I can speak it and write . But, I have never indicated Urdu as my native language. I always indicated English only.
