About my physical appearance changes

I did an operation to remove a mole on my face, which is about 4mm. But the mole is a little bit clear on the passport photo. Is there something wrong with me when I get into CA? Would the officers doubt my identity? Btw, my doctor gave me a certificate of disease. He said some patients who did cosmetic surgery used such documentation to ensure that they can go to foreign countries. But it wrote in Chinese, which has the doctor's signature and the seal of hospital.
