5409E - Declaration of Common-Law - dates?

Hi,

I am wondering what to put for the "to" date when it asks when you have been in a common-law relationship.
Does it have to be the date you get the form notarised? Can it be left blank if we're still living together?

I just wrote in present since we are still living together. Hope this helps!
