5406 Form Help

Help required in filling out form IMM 5406 ( Additional Family Information ) . The form says that all should complete their own copy of this form , since we are husband and wife I think we need to submit 2 copies , one which will be filled by me and one by my wife .

In Section A , it says Application where I wrote my name and under Spouse I wrote her name and details , later it asks for Mother and Father so I wrote my mother and father's details .

Now I am done with my section and If I ask my wife do to this , she will still write my name under applicant and her name under spouse section however she will write her mother and father's name in Section A , but how would CIC know that this form was filled by my wife or me ? just by signature ? . I am confused because in both the cases Applicant and Spouse name would be static .

In fact there is a confusion on this form among a huge number of applicants.
This form is however an additional information for family, so that there is nothing wrong as you already filled :

The applicant in one form will be spouse in other form. Each applicant should give only details of their family in separate forms.

Hi.

I have the same confusion. But what i did was i filled out 2 forms. In my form, i am the applicant; in my spouse's form, i am still the applicant.
