Is difficult for a person with an accepted refugee claim to apply for and obtain a U.S visa without pr? Like what are the acceptance and refusal rates? and what are the things they take into account when refusing an application?

Always comes down to the individual profile.
 
The applicants funds, employment status, previous travel history, if they entered Canada via the US, reason for travel, etc.
