However I am M1 while my wife is M3 could this be an issue or should I just go ahead and take an upfront medical.

According to what i have read about remedicals and extensions, its subject to your country of residence. The probability of you getting an extension is quite slim.
To be honest, I think you should wait to get a remedical request. However some have been doing upfront medicals but i have not heard of any of them getting a PPR afterwards.
might as well just wait.
Nothing is certain with how IRCC works. I for example got a remedical in May 2021, passed in June 2021 , Still have not heard anything from them and my new medicals is counting, over 5 months now.
I’m sure you will get an update soon, a lot of people have been getting remedicals lately
