Ya I believe so . Cause the only email notice that I got is for PAL, nothing really that was for MP. So I think you have to go check online account or ECAS every so often to see if you get MP.
The immigration medical is sent electronically by the doctors. Find a doctor near you and ask them what do they need. https://secure.cic.gc.ca/pp-md/pp-list.aspx
Usually, just your passport is enough. After the examination, they will give you a reference number. That document should be uploaded to your profile. In 10 days, the doctors would send your report directly to the IRCC. Based on the reference number, your report will be added to your profile.
Please note that it is valid for 12 months from the date of your medical exam.

No updates after that, gc key showing 72% . Estimated processing is july 4, 2021. Will it help if I give upfront medical?? Please help. Waiting for your valuable advises and comments. TIA

I wish I would have opted for upfront medical after PAL, May be processing would have been over by now.
Upfront medical exam:
Taking the exam before getting the Medical request letter from IRCC.
Expiration is 1 year.
Some clinics don't offer upfront.
Additionally, you need to upload your e-Medical sheet(you can get this from your Dr.)in Webform.
An benefit is it may make the total process short.
