Yes on that time, test dates weren’t available,,so thats why I submitted with expired ielts...
but i m not sure about the letter of explanation, my agent submitted or not....i only signed the forms & documents they want me to sign...

I am sure if rep did your file he must have attached some form of proof,in that case chill they won’t return your application

The above three cases you mentioned ,one is Aug 27 ADR -There may be some major doc incomplete or missing the reason why he received PAL late.
The other 2 just started their PR journey as they are OCT AOR and Sep AOR so if you also or be it any applicant goes and does upfront medical this soon their medical pass would also be before PAL,correct?

Yes, that is what I mean. Medical and eligibility should be proceeded separately and assessors are also different. The sequence of progress can be changed anytime.

I am not sure on this, I believe when you receive communication from IRCC at different times as your file progresses the bottom part of the letter mentions the office where your file is being processed presently.
