Upfront medical is valid option for both online and paperbased applications, however medical expires in 12 months and average processing time for paper based applications is 18 months. There is a chance your application not finalised within 12 months and in that case you have yo redo medical exam.
There is another option for upfront medical option, submit application normally and go for upfront medical after 5 to 6 months in that case there is possibility you will save 2 to 3 months in overall processing time for your application.

Now just awaiting the CIC portal to update with the confirmation of Biometrics being received. These were provided at a Service Canada, so I assume the process is near instantaneous and it merely takes a while for CIC to acknowledge them and update the portal. Anyone done biometrics yet and know how long it takes for the portal to show that step of acknowledgement?
After this I assume its ADR (if needed), and MR (a while down the line I presume)?
