No bio request I got additional document request dads death certificate as soon I send I got decision made and today got letter says forwarding file to New Delhi

I got a letter from cic in email saying that my application is incomplete. Cic wants me to submit additional family form imm 5406 for my mother .even though I submitted.
My father is primary applicant and mother is dependent. In imm 5406 for my mother , in APPLICANT section i have put my father’s name in already submitted form. Should i put my mother’s name in applicant in that form ? I think that is why cic miss understood with my mother’s previously submitted forms. I am requesting all of you please help me . I have 30 days to submit form again but need to know the fact. Thanks

Yes you should have submitted 2 copies. In one, your father as an applicant and in 2nd one, your mother as an applicant. Then, they both sign their respective forms.

That form is for the principal applicant and each dependant so yes your father needs to complete one to show his relatives

Yes applicant name should be mothers name .

IM5406 one for your dad and another for your mom.
for your father form IMM5406 = applicant name >> your dad name and his family details
for your mother form IMM5406 = applicant name >> your mom name and her family details.
