The Declaration of Common Law Union IMM5409, and Additional Dependents IMM0008DEP. These 2 forms are not applicable. Do I just not include them in the package, or include them with putting Not Applicable in all fields?

I am on parental leave right now and receive EI. For the Financial Evaluation Form IMM5768, for question 14, for the "present" line, what income do I put in the Gross Income/Benefits for period? Do I just add up all the income I've received so far from EI?

Also, for my husband, what income do we put for his current income since he is employed but this year hasn't finished? Do we just add up the total amount of salary he has received so far this year?

In present line is just total income you have received this year so far. Just make a total of your benefits to add there.
Same for your husband

I got the same paper size from my parents and the 5768 i printed here with the paper size that we use here.
Paper size does not matter.

Ditto for me. I have a mix of paper sizes in the application.

One line per employer/self employed for a period. Tottal for the period will thn equal noa 150.

My employments don’t match my line 150 as I have t5 slips which are investments. There is no place there to show them anywhere

So far it is a black box where no one knows what happen after you submit the information. On the letter of invitation, nothing is included basically about the parents or dependants.

Add T5 slip in the year earned. if you earned them in 2018 there will be two line for 2018 one for your employement income (01-01-2018 to 12-31-2018) and 2nd for T5 (01-012018 to 12-31-2018)

Can anyone please help about the additional family information form.

My mother has 6 siblings and i am going to attach an extra additional family information page to fill up the rest of the siblings.

Do i need to fill up only Siblings section ( Section C) or do i have to fill the full form again?

Just section c and your mother' name to show it is her details

There are some questions about the Common law in the financial evaluation form.
I am thinking to leave those section blank as i dont have any common law.

Some of previous thread advised NOT TO LEAVE ANYTHING BLANK..
I want to double check ---
SHOULD I LEAVE THOSE SECTION BLANK OR PUT N/A IN THOSE SECTION?

I need help regarding Surname column, my mother has not mentioned Surname in her Passport. Means Surname column is empty in passport. Now how to fill the Form because surname is required there.

I would scan all the form and email them to the principal applicant and the principal applicant signs them and courier it to me
