Look at the bottom of the attached document that came with the email. It is the same prohibition questions on the application form. Just print and sign it accordingly. Hope that helps.
Thanks but I got two attachment docs in my email. 1) Test Email content and 2) Test notice Feb 12 2018 document
Other than that I did not see any attachment. That is the reason I asked like how to do I get the form. in the first document they said bring the prohibition form but I did not see form. So I am confused like where can I get the form? should I call back or wait for physical mail?
I passed my test yesterday 20/20 from Edmonton and i don't have my old passports so they told me they will check with the service border to see all my entries and exits and this probably gonna delay my citizenship for more couple monthes, is anyone have similar experience would share you share please thanks

Received the test letter today. They have asked for:
- 2 pieces of ID one of which has photo and signature
-All original documents that support the citizenship application ( as listed in the document check list)
- All original passports and travel documents (current and expired )

From what I've read on the forum, sounds about right.
Sucks that they want ALL original passports, and not just the ones that cover the eligibility period. I'm debating whether to get this one stamp translated in my passport from more than a decade ago. Likely won't.
