When ur medical is approved is there specific time thAt they take ? Like someone said to me they take 3 months after the medical for passport request

I have question regarding Upfront Medical.
We did the upfront medical yesterday. I am the principal applicant and I have Dependent as well.
Do I have to submit through separate form when we submit the Medical results through Case specific enquiry? or can I use one form is enough to submit both results?
Please let me know if any one have experience on this/ Any suggestions.
