I have a question concerning the imm 5669 form
I have been asked to refill the imm 5669 form for my dependent for particular dates (from when he obtained his diploma till when he travelled to canada ( 7 months approximately). The problem is that he was on holidays and then waiting for his visa and did nothing very important during those 7months. Is it ok to explain those 7months by writing “holidays”?

Yes, you can. If he was in his home country, write "unemployment" for the activity and for the next column write "waiting for visa" as the explanation. Make sure there is no gap in time.

In the activity column, write "Unemployed"، for city and country write the name of the town and home country, for status write "Citizen" and in the name of company, employer, school, facility, as applicable column (the last column) write "Waiting for Visa".

If they're asking for dates, there is a great chance that you missed those dates in the original form submitted.
As I said, there should be no gap between dates. The end date of an activity should be the starting date of the other meaning they all should be in sequence with no gap in time.
You also need to include information up until the date you're filling the form (most recent information), i.e. until today if you're submitting it today.
Also, it should be complete, not just covering the missing dates.

No problem.
It depends, the officer will mention for whom you need to provide the form. In the request pdf, before the explanation begins there is the person's name and his/her UCI. For example it is something like this:
This is in reference to your application for permanent residence in Canada. In order for the processing of
your application to continue, the following document(s) are required:
<UCI> - <Applicant Name or Dependent Name>:
> Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature with updated personal, address and travel history since the age of 18 with no gaps in addresses or activities...."
So, you need to fill the form for the people that are mentioned there.
For your second question do as follow:
For dates, mention the start date as the date the implied status is started for your dependent, for end date write "today". For Activity write "Unemployed", for city and country use "<The city your dependent lives>, Canada", for Status write "Implied Status" and for name of company, employer, school, facility, as applicable column write "Waiting for work permit (mentioned Application #)/Waiting for PR application (mentioned Application #)"

Please Another question for a friend of mine.
His dependent was on implied status and then got refused her student/work permit in March (and she has 90days to restore her status)
Now they asked for another imm 5669 form for him and his dependent. The dependent has been unemployed from November till now and was on implicite status from December till March. Please What should he write on activity and status from November till present ? And what should he write on status from march (when the permit extension was refused) till now?
also, does he have to mention the extension refusal of his dependent on the imm 5669 form? (This was the first time she go a permit refused)
